Granite Alliance requires prior authorization (PA) for certain medications in order to ensure the most cost-effective and beneficial treatment course. We offer real-time PA determinations through CoverMyMeds - just submit your request for any medication through the portal and receive a determination within minutes.
Our prior authorizations are processed through our pharmacy benefit manager, Magellan Rx Management. You can access their secure CoverMyMeds portal below to submit a request. You can also submit a request using our coverage determination request form, however we strongly recommend using CoverMyMeds in lieu of the form for a faster response.
